How to add, remove and change order for form fields to collect different data

To add, remove, change order for form fields to collect different data:

Go to the component settings
Scroll down to the "Call to action form/button" (If necessary, switch to the form)
To add a field, use the corresponding button
To move or delete fields, use the arrows on the side of the fields and the button with the trash can symbol
